Safe Transitions is looking for a dependable and positive role model to work as a Mental Health Worker in an adult foster care home. Current openings in the Moose Lake area for Full-time, Part-time and Fill-in staff. Shifts available are varying and can include evenings, overnights and weekends.

The Mental Health Worker (MHW) provides direct services to clients in a manner that complies with corporation policies and procedures, government regulations, and client individual needs according to support plans. The MHW is directly responsible for the monitoring of client behaviors and program activities to assure the safety and well being of each client. The MHW is responsible for delivering the highest quality services possible.

Valid driver's license, high school diploma or equivalency, experience working with adults with mental illness preferred, and successful completion of the corporation new employee checklist and trainings, state and county background clearance. You may be required to drive your own vehicle if a company vehicle is not available.

Supervised by Program Director, Program Coordinator, and House Supervisor.

Supervises Implementation of client support plans, activities of clients, and activities of fellow MHWs.
